Warren County SNAP households that have been impacted by the April 18-19 snowstorm power outages now have until May 15th, 2022, to report the loss of food.
The LDSS-2291 REQUEST FOR REPLACEMENT OF FOOD PURCHASED WITH SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM (SNAP) BENEFITS form must be used to request replacement of food purchased with SNAP benefits. It can be accessed at: https://otda.ny.gov/programs/applications/2291.pdf.
